Prepaid Containerized Cargo Permits
Prepaid Containerized Cargo permit forms (O-forms) may be purchased for $50.00 each at the Truck Permit Office, the Louisiana Truck Center or by mail. These forms are for customers who do not have a charge account, but wish to purchase forms in advance of their use.
O-Forms can only be used for yearly Containerized Cargo permits and allow the same weights and are subject to the same conditions as those acquired by other methods (see page 28).
Blank Prepaid Containerized Cargo Permit forms must be activated within twelve (12) months from the date of purchase (as stated on the permit forms).
